If you need to update your name in Eastern University systems, please follow the process below.
Step 1 – Complete the Registrar’s Office Form
Fill out the Name/Address Change Form: https://www.eastern.edu/sites/default/files/inline-files/change_name.pdf
Step 2 – Provide Documentation
You must present:
A government-issued photo ID in your new name, and
One of the following:
Social Security card in your new name
A second government-issued photo ID in your new name
Original legal name change document (marriage certificate, divorce decree, or court petition)
Important Notes
Only legal name changes are processed (marriage, divorce, court-ordered).
Preferred names or requests for different email addresses cannot be accommodated by IT.
Once you submit the form and documentation, the Registrar’s Office will handle your request.
If additional IT updates are needed afterward, the Registrar will contact IT directly.
